PROMOTION DESCRIPTION: The "Smile Makeover Sweepstakes" ("Sweepstakes") is sponsored by Dental Arts Management, LLC ("Sponsor"). By participating in the Sweepstakes, each entrant unconditionally accepts and agrees to comply with and abide by these Terms and Conditions and the decisions of the Sponsor, which are final and binding in all respects. The Sponsor is responsible for the collection, submission or processing of entries and the overall administration of the Sweepstakes. Entrants should look solely to the Sponsor with any questions, comments, or problems related to the Sweepstakes.

ELIGIBILITY: Open to legal residents of Oklahoma who are at least eighteen (18) years of age at the time of entry. Employees of the Sponsor, its affiliates, subsidiaries, advertising and promotional agencies, and the immediate family members of each are not eligible to participate in the Sweepstakes. This Sweepstakes is subject to federal, state, and local laws and regulations and is void where prohibited.

SWEEPSTAKES PERIOD: Entries will be accepted online starting on May 1, 2024 and ending July 31, 2024. All online entries must be received by 11:59 pm on July 31, 2024.

HOW TO ENTER: No purchase necessary to enter or win. A purchase does not increase the chances of winning. Enter the Sweepstakes during the Promotion Period online by submitting the entry form which can be accessed by accessing the webpage:https://www.dentalartsofok.com/sweepstakes. Entries that are incomplete or do not conform to the rules may be disqualified at the sole discretion of the Sponsor.

USE OF DATA: All participants consent to the use of their submitted photos for marketing, advertising, or any other promotional purposes by Dental Arts Management, LLC.

PRIZES: The Winner will receive a $5,000 credit towards dental services provided by the Sponsor's designated dental clinic. No cash or other prize substitution is permitted except at the Sponsor’s discretion. The prize is non-transferable. Any and all prize-related expenses, including without limitation any and all federal, state, and/or local taxes, shall be the sole responsibility of the Winner.

WINNER SELECTION AND NOTIFICATION: Winners will be selected under the supervision of the Sponsor. Winners will be notified via email to the email address they entered the Sweepstakes within five (5) days following the selection of Winners. The Sponsor has no liability for a winner's failure to receive notices due to spam, junk e-mail, or other security settings or for winners' provision of incorrect or otherwise non-functioning contact information. If the selected winner cannot be contacted, is ineligible, fails to claim the prize within 15 days from the time award notification was sent, or fails to timely return a completed and executed declaration and release as required, the prize may be forfeited and an alternate Winner selected.

GENERAL CONDITIONS: The Sponsor reserves the right to cancel, suspend and/or modify the Sweepstakes, or any part of it, if any fraud, technical failures or any other factor beyond Sponsor’s reasonable control impairs the integrity or proper functioning of the Sweepstakes, as determined by Sponsor in its sole discretion.

LIMITATION OF LIABILITY: By entering, you agree to release and hold harmless the Sponsor and its subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents, successors, assigns, employees, officers, and directors from any liability, illness, injury, death, loss, litigation, or damage that may occur, directly or indirectly, whether caused by negligence or not, from (i) such participant’s participation in the Sweepstakes and/or his/her acceptance, possession, use, or misuse of any prize or any portion thereof.

DISPUTES: THIS SWEEPSTAKES IS GOVERNED BY THE LAWS OF THE UNITED STATES AND OKLAHOMA, without respect to conflict of law doctrines. As a condition of participating in this Sweepstakes, participant agrees that any and all disputes that cannot be resolved between the parties, and causes of action arising out of or connected with this Sweepstakes, shall be resolved individually, without resort to any form of class action, exclusively before a court located in Tulsa, Oklahoma having jurisdiction.
